Should I talk to my insurance company before my appointment?

Yes. Be sure to find out what services, tests, procedures your insurance provider will cover before your appointment.  Our physicians are Retinal Specialists; please check if a prior authorization or referral is required
before your appointment.

How can I submit a claim to my insurance provider or contact The Retina Institute if I have any questions?

As a courtesy, we are pleased to bill your health insurance.  Please provide our office with your insurance information by calling our Patient Accounts Department at (314) 367-5662 or (800) 888-0011 extension 2296. Our department hours are Monday- Friday, 8:30am - 5:00pm.  Our billing email address is:

Why did I receive separate bills for my visit?

You may receive two or three bills for the same hospital or surgical center visit.  However, these bills are for different services, you are not being charged for the same services.  A statement will come from our office for the physician’s fees. One bill is from the hospital or surgical center and covers the hospital or surgical center facility fees.  Another bill may come from an anesthesiologist for any anesthesia-related services.  

What if I cannot pay my bill in-full when I receive my statement?

We would be happy to set you up on an interest free, affordable monthly payment plan for certain services not including drugs.  Please call our office for more information and to discuss your options.


How do I obtain my medical records?

You can download medical record forms from this website, or pick-up them up at any of our offices.

Once completed, the forms can be mailed to:

To expedite a records release, completed forms can also be faxed to 314-367-1628; however, Health Information Services still requires release forms be mailed to the office so the original signatures are on file. No patient records can be sent electronically, such as via e-mail.
